Reglement

§1 General provisions

1. These Regulations set out the rules for providing rehabilitation, hotel and catering services at Ośrodek Rehabilitacyjny Leśna Polana sp. z o.o. with its registered office at ul. Leśna Polana 1, 72-300 Rzęskowo.

2. Making a reservation for a rehabilitation programme is equivalent to accepting these Regulations.

3. These Regulations apply to all Guests and Patients staying at the Centre.

§2 Reservations and payments

1. A programme reservation can be made:

• by phone at: 91 385 67 00
• by email at: rezerwacje@lesnapolana24.pl

2. A deposit of 30% of the stay value is required within 7 days of making the reservation.

3. The remaining amount is payable no later than on the day of arrival.

4. We accept payments by card, BLIK, Przelewy24, traditional bank transfer, through foundations and the MediRaty instalment system. Online payments are processed by FlashAI sp. z o.o. as a payment agent (Art. 758 CC).

5. A service fee may be added to online payments, visible at the time of placing the order. Transactions are processed by Stripe Inc.

§3 Cancellation and refunds

§4 Arrival and accommodation

1. The hotel day starts at 14:00, and ends at 11:00.

2. On the day of arrival, the Patient is required to:

• presenting an identity document
• presenting current medical documentation
• completing the patient information card
• signing consents for data processing and treatments

3. Early check-in or late check-out is possible upon arrangement with reception, subject to room availability.

§5 Stay rules

1. A total ban on smoking tobacco and e-cigarettes, applies throughout the Centre, except in designated outdoor areas.

2. Quiet hours are in effect from 22:00 to 6:00.

3. The Patient is required to attend treatments punctually in accordance with the established schedule.

4. Failure to attend a treatment without prior notice results in forfeiture of the treatment without the possibility of rescheduling.

5. Guests are financially liable for any damage caused to the Centre's property.

6. For fire safety reasons, the use of heaters, irons and other devices not included in the room equipment is prohibited.

§6 Rehabilitation services

1. The rehabilitation programme is determined individually by the doctor after an initial examination.

2. The Patient has the right to information about planned treatments and their purpose.

3. All ailments and concerning symptoms must be reported to medical staff immediately.

§7 Meals

1. Full board is included in the programme price (3 meals a day + 24h coffee/tea buffet).

2. Special diets (diabetic, gluten-free, vegetarian) must be requested at the time of booking. Specialist diets are subject to an additional charge.

§8 Final provisions

1. In matters not regulated by these Regulations, the provisions of the Civil Code shall apply.

2. These Regulations come into effect on 1 January 2026.
