Everything You Need to Know Before Arrival

We have prepared a complete guide for patients and their families. Here you will find information about documentation, equipment, fees, and the rules of stay at Leśna Polana.

Do I need a referral?
No, a referral is not required for commercial stays and rehabilitation stays. A referral is only necessary for stays funded by NFZ or PFRON.
What documents should I bring?
ID card, medical records (discharge summaries, test results), current medication list, and disability certificate (if applicable).
Can I pay by card?
Yes, we accept payments by card, bank transfer, and cash. Reserving a rehabilitation stay requires a 30% deposit.
Does the centre organise transport?
Leśna Polana does not provide medical transport services. We recommend using transport companies in your region. Szczecin-Goleniów Airport (45 km) and Gryfice station (5 km) are well-connected.
Can I visit my loved one?
Yes, visitors are welcome. Please sign the visitor log at the nurse's station.

What do we offer?

As part of its inpatient services Ośrodek Rehabilitacyjny Leśna Polana sp. z o.o. provides round-the-clock nursing and care services and rehabilitation for individuals who do not require hospitalisation.

The purpose of the stay is to improve health, prevent complications resulting from the disease process and immobility, activate and improve mobility, and ensure safety and care.

Admission restrictions

The centre does not admit patients whose primary indication for care is cancer, mental illness or addiction.

Fees

  • The monthly stay fee depends on the scope of services ordered
  • Rehabilitation programme reservation is finalised with a deposit of 30%
  • No advance payment is collected for inpatient stays based on a civil law contract
  • On the day of admission to the centre, the contracting party pays in accordance with the applicable price list
  • Payment methods: bank transfer, payment card, cash
  • Additional services are settled at the end of the billing period
  • A deposit is collected a deposit of min. 200 PLN for unforeseen patient expenses

No referral required

You do not need a doctor's referral to attend a rehabilitation programme.

Patient admission

Admission of the Patient to Leśna Polana takes place upon:

  1. Signing the contract and appendices
  2. Signing the consent for the stay
  3. Payment for the stay (cash/transfer/card)

Admission date is set individually. Stay rates are calculated on a per-day basis, so charges begin on the day of the Patient's admission, regardless of the time of arrival.

On the day of admission, the patient is placed under 24-hour staff care, is examined, accommodated and fed according to recommendations. A stay plan is also established, including rehabilitation hours, which begin on the second day of the stay.

Patient's personal equipment

On the day of admission, the patient should have:

Clothing

  • Pyjamas / nightgown
  • Dressing gown
  • Tracksuit or sportswear
  • Daywear with socks
  • Sturdy footwear

Hygiene

  • Oral hygiene supplies
  • Liquid soap, shampoo
  • Disposable washcloths, 2 towels
  • Wet wipes
  • Body lotion/balm, sudocrem

Medical

  • Incontinence products for the entire stay
  • Medications for the entire stay
  • Identity document (for verification)

Equipment (if owned)

  • Wheelchair with belt
  • Walking frame
  • Walking cane
  • Personal items (photos, cards)

On-site shop

Most essential products are available on-site, including incontinence products and professional Seni care products.

Stay rules

The Patient and their caregivers are obliged to comply with the Regulations, and in particular to:

  • Polite conduct towards staff and other patients
  • Observing quiet hours (quiet hours 22:00 - 06:00)
  • Remaining sober and refraining from alcohol consumption
  • No smoking (a strict ban applies throughout the entire centre)
  • Taking care of the centre's and other patients' property

Important restrictions

  • Lifting patients, extra feeding or giving fluids without staff knowledge is prohibited
  • The centre is not liable for patients' belongings and valuables
  • Cash, cards, jewellery should be deposited in the safe or left at home

Providing information

Information about the course of the stay and rehabilitation is provided to exclusively through personal contact the authorised person (designated at the time of signing the contract).

In life-threatening situations

The emergency services are called to the centre. The paramedic decides whether the patient remains at the centre or is transferred to hospital. The family is notified after the emergency visit is concluded.
